NHS number and birth certificate

Dear Parent / Guardian,

Our records show that you have not yet provided us with your child’s NHS number or a copy of their birth certificate. It is important that this information is provided as soon as possible.

Please provide the HNS number below. If you do not know your child’s NHS number it can obtained by calling your doctor’s surgery.

You can also attach a photo or scan of their birth certificate below.
